Forms needed to apply for Learner License (Permit)
DE-964 - this form will be given to the student after 6 hours of class has been completed and have passed the Permit Test.
DL-14B - Application for Texas Driver License or Identification Card
VOE - Verification of Enrollment from High School your teen attends. This form can be picked up at the schools Main Office.
Birth Certificate - State certified only with the raised seal. No copies!
Social Security Card - signed original card

Forms needed to apply for Driver's License
- The Teen must be 16 years old
- The Teen must have held their permits for 6 months.
-Plastic Permit
-Parent Log
- A New VOE
-DE-964 this form will be given to the student after completion of the full course.
-DL-14B - Application for Texas Driver License or Identification Card
- $11 to process driver's license
-A Car with insurance ( the teen does not have to be on the insurance until after they have been issued a driver's license)
- ITTD (Impact Texas Teen Driver completion certificate)
